Description
| Description (Service) | Provides support to adults with developmental disabilities in leading lives as near to normal and as independent as possible, within a community setting. Provides case management, including assessment, coordination, advocacy, counselling and referral to other services. The majority of visits are provided in individual's home or at various locations in the community. |
