Description (Service) Helps individuals to maintain their independence despite illness, injury, disability or age offering the following health services:

24 hour nursing services - visiting and shift services available in home, school, or at clinic including:
  • wound care
  • ostomy management and teaching
  • palliative care
  • home risk assessment
  • chemotherapy
  • ventilator management
  • medication management
  • chronic disease management
  • post surgical care
  • care of fragile elderly or medically fragile and technologically dependent children
Rehabilitation services provided in homes, workplaces and schools including:
  • physiotherapy, PT
  • occupational therapy, OT
  • speech-language therapy
Nutrition assessment and counselling by dieticians for a variety of medical conditions including diabetes management and weight management. Offers nutrition education and support associated with cancer therapy. Monitors feeding and growth of infants and children and offers support for clients on enteral feeding and total parenteral nutrition.

Home assistance with daily living activities such as:
  • personal care (bathing, dressing, personal hygiene)
  • medication reminders, pre-measured use and monitoring
  • mobility
  • light housekeeping
  • laundry
  • shopping
  • meal preparation
In-person clinics available at Kincardine, Walkerton, Wiarton and Owen Sound. Check website for location or call for information.
