|Description (Service)||A three-month exercise and counselling program for adults who have had a heart event in the past two years. The supervised program takes place at the YMCA and a home based program is available for those that are eligible.
Assessment is done by a Registered Nurse, and if necessary the program Medical Advisor.
Individual exercise guidelines are based on stress test results when available and monitored by the YMCA Cardiac Rehab Exercise Specialist.
Joint program of Grey Bruce Health Services and YMCA of Owen Sound Grey Bruce.

Adults who have had a heart concern within the last two years (for example heart attack, bypass surgery).
|Application||Medical referral required. Physician or cardiologist first makes a referral to the program, individual will then be contacted by office for an appointment. Referral found on www.gbhs.on.ca|
