|Description (Service)||COVID19 Assessment Centre available at Grand Bend clinic, 69 Main St. E (see LAM0082) by appointment only. Call or book online HERE Drive-up available. People without vehicles will also be accommodated. Self-referral accepted by appointment. Testing only.

|Eligibility||Residents of the catchment area requiring medical services.|
|Application||Patients requesting a family doctor at GBACHC fill an application and must live in our catchment area; Grand Bend, Hensall, Crediton, Dashwood, Kippen or Port Franks.
Applications are triaged by a registered nurse and the patient will be contacted when there is an opening.
|Languages||English ; We have capacity to arrange for interpreter services if necessary. Please ask.|
|Fees||None ; Ontario residents will need their valid OHIP card, residents of other provinces must have their Provincial Health Card. U.S. or Foreign visitors will be asked to pay cash for their visit.|
