| Description (Service) | Description: Community Participation Supports offers an individualized, strength-based approach to person-directed planning that promotes meaningful and inclusive community involvement. Under this umbrella, three programs are available: Passport Administrative Services, Foundations Huron, and Employment Supports. Passport Administration Services: Passports is a program that helps adults be involved in the community and live as independent as possible by providing funding for community participation services and supports, activities of daily living and person directed planning. Individuals in receipt of Passport funding choose their Community Mentor, who provides 1:1 support to work towards individualized goals to build skills, and to connect to natural supports in the community. Foundations Huron: Provides the opportunity for young adults to make a successful transition from high school to adult life in the community. The program provides Person Directed Planning, support, and encouragement to develop future life choices. Goal oriented supports are provided by a Community Mentor. Benefits to the participant are: setting and achieving goals, developing personal and life skills, increasing social, volunteer and employment opportunities, and building natural supports. Employment Supports: Employment Supports provided job coaching and job readiness for competitive employment and traditional volunteer work in the community. Support is provided by one or more skilled Employment Support Job Coaches. Job development, job coaching, skill enhancement, employer liaising are examples of some of the support areas. Eligibility: Adults, 18 years(s) and up, with a developmental disability. Eligibility for Ministry Passport funding, Foundations Huron & Employment Support is determined by Developmental Services Ontario (DSO) at 1-855-437-6797. |
