Description
Description (Service) Primary health care team that brings together different health care providers to coordinate patient care in order to assess and treat injuries and/or illnesses. The team include family physicians, nurses, nurse practitioners, social workers, dietitian, and pharmacist.

Cholesterol Management
Diabetes and prediabetes program - clinic on Thu
Travel vaccine assessment
Medication assessment and review
Lung health, asthma, chronic obstructive pulmonary disease (COPD)
18 month well baby program
Geriatric (older adults)
Oncology nurse navigator
Craving change program, healthy eating workshop series
Seasonal flu vaccine
