Description (Service) | Interdisciplinary team provides services and supports for older adults, their families and caregivers who are coping with or at risk for developing responsive behaviours associated with dementia, mental illness, addictions and other neurological conditions. Services include:

Eligibility | Ages: 65 year(s) and up Adults 65 years and older (unless dementia diagnosis is present) with responsive behaviors related to mental health, addictions, dementia, and/or related neurological conditions, living in long-term care homes or in community settings, and their caregivers. |
