Eligibility For Primary Care Services:
  • must enroll to have a Family Doctor or Nurse Practitioner
  • waitlists are in place for allied health services and priority is based on condition
Must have a referral from health care provider for physiotherapy
Unless otherwise noted, all other programs and services are available to all community members
Application For Primary Care Services (This is not a walk-in clinic.):
  • For new clients, submit a form called "Request for a Family Doctor or Nurse Practitioner" to have primary care at the Community Health Centre. Note: Roster of patients is full at this moment and waitlist is closed.
  • Rostered patients can phone to make an appointment
  • Same day /urgent care appointments are available for rostered patients.
For Community Programs and Services:
  • Walk in or call to register or inquire about community services and programs, some programs now require pre-registration.
  • Volunteers need to complete a volunteer application form and a criminal reference check/vulnerable sectors screen.
Languages Interpretation services can be arranged in advance, and sometimes on-demand if interpreter is available.
