| Description (Service) | COVID-19 Response Until further notice, all group and educational programs at the Hanover Family Health Team are discontinued * Nutrition and counselling staff are working from home and are providing telephone and electronic appointments * Other staff members are now working at the Hanover Medical Clinic * Routine medical and occupational therapy appointments are not being accepted at this time * Please also refer to the website for updates Regular Service: Provides primary health care, including social, addictions and mental health services, chronic disease management, health promotion and disease prevention to rostered patients in the town of Hanover and surrounding area * Team Includes family physicians, a nurse practitioner, registered nurse, addictions/mental health counsellor, registered dietitian, psychologist, pharmacist, occupational therapist and social worker Services and programs offered include: prenatal classes * cooking for 1 or 2 * medication review * quit smoking * COPD and asthma clinic * hypertension * diabetes * nutrition * women's health * anxiety and depression management * chronic pain management * living on less * cardiac rehab * 3 year old well child * memory clinic * osteoporosis * pulmonary rehab * breastfeeding and infant feeding support Individual, couple and family counselling, as well as individual health assessments and treatments, are available by appointment and by referral for rostered patients of Hanover physicians Also offer health promotion, wellness and disease prevention educational group programs for anyone in the community, no referral required |
