Description

Description (Service) Provides assessment, consultation, treatment and support to patients and families living with a dual diagnosis (people who have a diagnosed developmental disability and are also experiencing a mental health problem such as a serious mental illness, aggression, self-injury).

Program staff and a consulting psychiatrist collaborate with mental health and developmental service organizations to support:
  • planning and evaluation of treatment strategies
  • coordination of services
  • referrals to services in the community
Services are available in Grey and Bruce Counties in any location where persons with dual diagnosis reside, such as a private home, group homes, hospitals, nursing homes.

Runs a dedicated outpatient psychiatric clinic once a week.

Eligibility Ages: 18 year(s) and up Adults with a developmental disability and a suspected mental health need

Application Self-referral. For a psychiatric appointment, a referral from a primary care provider is required.
