| Description (Service) | Family health care team brings together different health care providers to coordinate patient care. Providers include:
Offers programs such as smoking cessation, diabetes management, Long COVID and memory clinics. Mental health programs for children, youth and adults also available (see separate listing). After hours clinic for urgent consultation. Available only to patients of Owen Sound Family Health Team doctors. |
