Description

Description (Service) The team with the Geriatric Assessment & Intervention Network works with frail seniors and their families to help keep them living safely and independently in their own homes.

With the client and family members, the GAIN will:
  • Complete a comprehensive assessment
  • Develop a personalized care plan
  • Provide follow-up in the home
The GAIN team members include: behavioural support nurse, care co-ordinator, client services worker, community health worker, nurse practitioner, occupational therapist, office support, pharmacist, physiotherapist and a social worker.

Eligibility GAIN is accepting referrals for individuals generally age 65 or older, who:
  • Are living at home or in a retirement home
  • Have complex medical problems or medication issues
  • Have a recent unexplained decline in health and/or level of function
  • Have a loss of capacity for independent living
