Description (Service) | The team with the Geriatric Assessment & Intervention Network works with frail seniors and their families to help keep them living safely and independently in their own homes. With the client and family members, the GAIN will: * Complete a comprehensive assessment * Develop a personalized care plan * Provide follow-up in the home The GAIN team members include: behavioural support nurse, care co-ordinator, client services worker, community health worker, nurse practitioner, occupational therapist, office support, pharmacist, physiotherapist and a social worker. |

Eligibility | Ages: 65 year(s) and up GAIN is accepting referrals for individuals generally age 65 or older, who: * Are living at home or in a retirement home * Have complex medical problems or medication issues * A recent unexplained decline in health and/or level of function * Loss of capacity for independent living |
