|Description (Service)||To provide case management and advocacy for adults who have a developmental disability. Services include: assistance with money management, developing access to specific community services, support for resolving legal issues, medical and dental case coordination, individualized support (short term), emotional support, and long term planning.
Tri-County Community Support Services are closed from March 16th to April 6th 2020
Mon-Fri 8:30 am-4:30 pm up to 21 hours per week

Deemed eligible and referred by Developmental Services Ontario (DSO) under the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act, 2008.
Residing within the HKPR area
The APS program is primarily intended to support individuals who live substantially on their own but the APSW guidelines allow other eligible individuals to receive support for a limited time, typically up to a year. This can include individuals living in residential programs or with family members.
