| Description (Service) | The Palliative Care Community Team is a partnership of Haliburton Highlands Health Services, Haliburton Highlands Family Health Team, Central East Local Health Integration Network and Peterborough Hospice Palliative Pain and Symptom Management Consultation Services. Our highly skilled, professional and compassionate Palliative Care Nurse Navigator becomes your care partner and link to the health care team to ensure timely and ongoing implementation of: shared care plans, assessment, referral, access to palliative care, grief, bereavement and support services |
