Description (Service) | Delivers centrally coordinated, individual support and advocacy services in addition to community based programs to Métis Communities in Ontario * support services are client-driven, family centered and provide holistic care Healing and Wellness Programs across the lifespan: Métis Healthy Babies Healthy Children Program: Supporting caregivers with children 0-6 years. Métis Family Well Being Program: Supporting youth and families. Community Wellness Program: Supporting Adults. Community Support Services: Supporting seniors and those with chronic illness. Aging at Home: Supporting Métis over 55 years. Access to Mental Health and Addictions Services through Ontario Telemedicine Network (OTN). |
