Description

Description (Service) Transitions homeless individuals or at risk of homelessness to become self sufficient and improve the quality of their lives through:
  • housing
  • connections support
  • opportunities
Operates Hartog House, a 20 unit transitional housing complex
  • provides temporary housing for up to four years.
Runs onsite programming to develop life skills, reduce isolation and build a sense of community.

Provides case managed support services to identify short and long-term goals with an aim at helping individuals to:
  • transition to long-term and permanent housing
  • self-sufficiency and independence

Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Christian Island ; Midland ; Penetanguishene ; Springwater ; Tay ; Tiny
Eligibility Individuals 18 years and older and families experiencing homelessness or at risk for homelessness and who have been prioritized through the Coordinated Access system.
Application Referral from Simcoe County Coordinated Access system.
