Description

Description (Service) Program offers:
  • mental health case management
  • care and support for individuals living with a mental health disorder or concurrent disorders
  • shared care format working with other Indigenous and non-Indigenous service providers
  • Indigenous cultural approach built on Indigenous best practices
Services include but are not limited to :
  • family consultation
  • counselling and psychotherapy
  • CANS and OCANS assessment
  • care planning
  • housing supports
  • opioid supports
  • referral to other appropriate services.
Services are available for Indigenous families affected by mental health challenges and support for children, youth and adults needing after care.

Community capacity building, training, service system liaison and networking are available for other service providers.
