Description
Description (Service) A Mobile Support Team responds to calls from individuals and agencies in the community and to staff in long-term care when there are behaviours that they can no longer manage with the goal to safely manage the person with behaviours in their home. Behaviours may be associated with cognitive impairment due to dementia, neurological disorders, mental health and/or addictions. 
 
The MST will enhance existing resources in a model of collaborative care with input and direction from the client and carers (family or care providers) Length of support: short term (up to 6 or 8 weeks)

Areas Served Muskoka District ; Simcoe North
Eligibility Older adults living in the community or in long term care
Application Care providers with concerns about a person's behaviour in the community or a long-term care facility call the Waypoint central intake number for referral to Mobile Support Team. Physician referral not required.
Languages English ; French - Interpretation, By request, Informal
