  • Work collaboratively with care partners and patients to develop, implement and evaluate a behaviour plan of care

Eligibility Older Adults with cognitive impairment (could be due to a dementia, neurodegenerative disease, mental illness and/or addictions), evidence of responsive behaviour (defined as actions, gestures and/or words that occur in response to a real or perceived stimulus such as hitting, kicking, rummaging, resistance to care, wandering, exit seeking, sexually responsive behaviour, etc.).
Application Care providers with concerns about a senior should call the central intake number. Physician referral not required.
