Eligibility	Priority reserved for Seniors residing in the North Simcoe for the past 5 years 65 years and older with an emphasis on those who are 75 years and older At risk for falls including orthopedic trauma Experiencing difficulties with managing day to day activities Experiencing some difficulties with cognition as a result of the aging process Marginalized Socially and geographically isolated Low Income - less than $2000 per month Assets not over $50,000
