| Eligibility | Adults with limited independence (age or disability) and adults waiting admission for long-term care |
| Application | By referral - client and caregiver are invited to call or to visit the program for an assessment of the clients, goals, needs, likes, dislikes and abilities. Home and Community Care Support Services - North Simcoe Muskoka also assesses needs of all applicants. |

| Fees | Service - Day Program - $22 per day ; Fee assistance available for individuals with limited income ; assistance is provided by County of Simcoe. |
