Description (Service) | COVID-19 Response During the COVID-19 Pandemic, the office will be closed to the public until further notice * Appointments will be conducted virtually (by phone or video) * All group programs are temporarily on hold
Regular Service: Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family physicians, nurse practitioners, registered nurses, mental health therapists, respiratory therapist and dietitian Programs and Specialty Care Teams: Hope Huntsville Palliative Care: Provides quality end-of-life care to patients and families with an advanced progressive illness Geriatric Care: * Provides a comprehensive assessment and assists seniors with complex health problems. * family and caregiver support related to senior's care including: depression * dementia, confusion * memory * falls * medication problems * has a geriatric outreach team to provide care in home Pulmonary Rehab/Asthma/Smoking Cessation: Offers self-management programs for people and their families who live with asthma or COPD (Chronic Obstructive Pulmonary Disease), as well as smoking cessation. Dietitian services: Offers assistance to patients with: * Weight management * Elevated cholesterol * Diabetes prevention * Irritable bowel disease and other digestive disorders * Childhood nutrition * Sport nutrition * Healthy eating on a budget Mental Health Therapy services: Counselling program offers short term counselling to adults, children, and youth for issues such as: * Anxiety * Depression * Grief * Couple issues * Stress Primary Care services: Registered Nurses and Registered Practical Nurses offer chronic disease management and health promotion programs in the doctor’s offices of the Algonquin Family Health Team.
