| Description (Service) | Provides support to children impacted by a developmental disability, utilizing skilled staff and effective partnerships. Special Needs Resource Teacher Program Serves children 2-6 years who have one or more delays in their development and who attend a licensed childcare center or home childcare. Provides case management services to assist families in accessing services and supports for their child. Family Support Program Supports families who have school aged children who have a developmental disability. Assists families to access resources to enhance their child's development and ensure they can fully participate at both school and in their communities. The Family Support Worker acts as a case manager, coordinating resources and assisting in the development of an individual family support plan including both life and school transition goals. Children's Respite Services Provides primary caregiver of a child with a developmental disability with a break from caregiver responsibilities. Can take place in a variety of settings and is flexible in time from a few hours to overnight breaks. |
