| Description (Service) | Provides support services for individuals with physical disabilities as well as frail, elderly seniors, and those living with age related dementia. Programs and services include: 24 hour Attendant Care for individuals living at the barrier free Forest Hill complex in Parry Sound and in apartments in Muskoka. Outreach Program - personal support and homemaking services for people living in the community. Clients must be 18 years or older so they can take legal responsibility for their own care. Respite Caregiver Relief services include: supervision, meals and activities, in the persons home or at the Forest Hill Apartments. Service is available on a daily, 24 hour basis. Adult Day Away Program - provides socially based gatherings for seniors or physically disabled residents in many communities focusing on the likes and abilities of each individual participant. Transportation to the programs may be able to be arranged. The programs are held from 9 am-3 pm, call for locations and times. Life Skills Program to help clients set and achieve their own goals. 24 Hour Assisted Living provides personal care, light housekeeping, meal preparation, security checks. Caregiver Support Program offers education, peer support, group and one on one support. A member of the North Simcoe Muskoka Community Support Services Coalition. |
