| Description (Service) | A collaborative effort with community partners to provide visible, accessible, and reliable health promotion and healthy living information, education, and programs to the community. Programs and services include: Healthy Heart Program: Provides educational sessions to assist those with or at risk for heart disease to improve their lifestyle and prevent future illness. Eight week exercise component that provides cardio and resistance training. Nurse Practitioner (NP): The NP provides general primary health care services for patients of all ages, and are involved in a variety of programs Diabetes Prevention Program: A free program for adults who are at high risk of developing Type 2 diabetes, with the following risk factors: overweight, especially in the stomach area; high blood pressure; high cholesterol; low physical activity levels; Diabetes during pregnancy. Breastfeeding Support Clinic: Free Breastfeeding Support Clinic with lactation consultant held once a week. Other programs and services offered at Howland Building location include:

Self referral. Diabetes prevention program is open to public. Non FHT patients may be accepted at the breastfeeding support clinic depending on availability. Must be a rostered patient of the FHT for all other programs.
