Description (Service) supports independent living older adults and their caregivers * in-home help and care * Meals on Wheels * transportation * home safety services * wellness, social programs and diversity outreach programs for isolated older adults * caregiver support and education * hospital-to-home transition * home medical equipment * Lifeline personal response system

Tenancy Risk Reduction Service : supports low and moderate income York Region older adults (or those with an age-related condition) at risk of losing their tenancy due to hoarding behaviour, cluttered living spaces, uncleanliness and squalor, and/or unmaintained homes

Transportation for clients living in own homes or retirement residences * escorts clients to and from medical appointments, Adult Day Programs, grocery stores, errands, CHATS programs, and more to facilitate their independent living * wheelchair accessible van available ONLY in Bradford West Gwillimbury and New Tecumseth

Transportation services are available only to clients living in their own homes or in retirement residences, not for those living in long-term care residences
