| Description (Service) | Delivers primary health care through a team approach with doctors, nurse practitioners, registered nurses and mental health workers. * Focus of care includes health care programs and services supporting healthy living and chronic disease management, of, for example, diabetes, high blood pressure and chronic pain. * Ten physicians, three nurse practitioners, three mental health workers, and support staff work out of five locations, Campbellford, Hastings, Warkworth, Colborne, and Havelock. |
