Description (Service) COVID19 Home and clinic visits available by appointment only. * Drop off of supplies or medications to seniors at risk. * Will continue to monitor all clients via telephone call and ensure proper care if there are any urgent issues or concerns. 
 
Provides comprehensive assessment, care planning and deliver, for frail older adults experiencing * changes in support needs, * multiple complex health concerns, * safety concerns or cognitive decline, * psychosocial concerns, * at risk or have experienced falls, * difficulty coping or isolation, * functional decline, * Medication assessement. 
* Inter-professional team works with older adults and their families to develop care plans that will support them so they are able to continue living at home.
