Description (Service) Service is offer in three ways: 
* Individual diabetes counseling 
* Team based services  
* Group educational sessions 
 
Topics that are covered include: 
* Myths and facts about diabetes 
* What foods will affect your blood sugar 
* How to read labels in the grocery store 
* How to plan for and incorporate more regular physical activity into your daily life
Service Details
Hours Office: Mon-Fri 9 am-5 pm 
Learning to Live with Diabetes: monthly, on Wed for 3 consecutive weeks 
* 2 hour sessions
Areas Served Orillia and area
Eligibility Must be referred by local family doctors that are a part of the CFHT or the Orillia Family Health Organization Physicians (OFHO) 
 
Learning to Live with Diabetes: A referral is required from your physician or from a member of the Diabetes team
