Description

Description (Service) Transitional housing program supports individuals with a developmental disability.
Offering a supportive living environment opportunity, and tools for skills development. Individuals in this program will use the steps to independence to document and identify the areas they want to learn or grow.

This is a two-year program for individuals not associated with Empower Simcoe and a
one-year program for individuals currently supported by Empower Simcoe.

Eligibility
  • Person is confirmed eligible by Developmental Services Ontario (DSO)
  • Shall have a diagnosis of an intellectual disability
  • Must be 18 years of age or older
  • Person has completed ADSS and SIS process
  • There are identified goals to learn some life and living skills
  • Application
  • Program will be accessed through different referral sources and community partners.
  • Community partners would submit names of clients for placement considerations.
  • Empower Simcoe's Quality and Innovations Resource Advisor, in collaboration with Clinical services team, will screen the list of candidates suitable for admission.
  • Based on the referrals received by Quality and Innovations Resource Advisor, Empower Simcoe will determine which individual is the most suitable for this setting and will work with the referral agency and other appropriate stakeholders in transitioning and admitting the individual to our service.
  • The Quality and Innovations Resource Advisor will ensure the following takes place during the intake process. Intake interview, information collections, review and sharing and risk protocol development and approval.
