Description (Service)

The Foundations Program supports young adults transitioning from student life to active participation in their community. Designed for individuals aged 18–26 living with a disability in Perth County, the program is accessed through referrals from Developmental Services Ontario.

Participants collaborate with trained service providers to build meaningful, inclusive, and self-directed lives. The program offers opportunities to strengthen skills in literacy, numeracy, and technology while working within a personalized plan. Focus areas include community engagement, self-advocacy, social connections, employment or volunteer support, and effective use of local resources.

Participation in the program is voluntary, confidential, and tailored to each individual's unique goals and aspirations.
