The Familyhome program provides individuals who have a developmental disability with the opportunity to live within a family. Familyhome provider recruitment, case management, care management, coordination, respite services

Hours Office: Mon-Fri 9 am-5 pm. On Call Service: Mon-Sun 24 hours

Eligibility Ages: 18 year(s) and up Adults with a developmental disability
Age Requirements: 18 year(s) and up;
Application Medical referral required. Applicants assessed and then placed on a waiting list for placement
