Description (Service) | The Clinical Intensive Case Management provides time limited, goal specific in home community treatment and support to individuals experiencing mental health and addiction issues. The program focusses on relapse prevention and stabilization in the community. Services include, but are not limited to mental health monitoring, medication management, psychoeducation, liaising with partner agencies and advocacy. |

Persons whose service provider practices in Huron or Perth Counties. Mental Health consumers not able to receive support by existing community resources. Individuals whose participation in program is voluntary. |
