The team is comprised of family physicians, nurses, nurse practitioners, physician assistants, dietitians, occupational therapists, mental health clinicians, and administrative support staff. The multidisciplinary team utilizes their varied knowledge, skills, and experiences to deliver the very best care to their patients. They offer various programs, services, and groups to their patients that focus on education, health promotion, and chronic disease management. |
