| Description (Service) | Primary health care team that brings together different health care providers to coordinate patient care, including nurse practitioners, nurses, pharmacists, dietitians, a psychological associate and mental health counsellors. Provide group, family and individual counselling Programs and Services: Bone Health, COPD Clinic, Diabetes 101 - Understanding the Basics, Diabetes DYAD Clinic, Feeding Your Baby, Flu Shot Clinic, Heart Health Workshops, Maternal Care, Persistent Pain management and Support Group, Raising a Healthy Happy Eater, Smoking Cessation, Sports Medicine, Wellness for Tots, Cognitive Behavioural Therapy for Insomnia (CBT-I), Creative Flow, Happiness 101, Intro to Mindfulness, Memory Clinic, Mindful Eating, Mindful Moments (Mindfulness Drop-in Clinic) and Mindfulness-Based Cognitive Therapy. The STAR Family Health Team offers an After Hours Clinic that rotates between their three clinic sites. Clinic is by appointment only for urgent problems that arise after 5 pm that day and cannot wait until the following day.
