If case manager responds in person to a client situation, symptom screening will take place. The response by CMHA staff may not be the assigned case manager. Toll-free 1-888-875-2944 and dial 0 for reception CMHA manages affordable rental units throughout Huron and Perth Counties, includes owned buildings and sublet units from local landlords. Tenants with limited income and evidence of a mental illness may qualify for a rental subsidy based on income * support services are provided to assist tenants to maintain their residences * long and short term accommodation is provided Transitional units provide temporary furnished accommodation to facilitate discharges from hospital and assess a person's skills to maintain permanent housing |

| Eligibility | Ages: 16 year(s) and up Youth and adults 16 years and older. Supportive housing is governed by existing operating agreements with the Ministry of Health and Long Term Care. CMHA Huron-Perth provides accommodation for individuals/families with serious mental illness who are homeless, at risk of homelessness or are paying more than 50% of their income on shelter. Rent payments are based on a calculation of tenant income. |
