Description (Service) If you are in immediate crisis, go to the Emergency Department or contact the Four County Community Mental Health Crisis Response Program at 705-745-6484 or 1-866-995-9933 * 24 hours a day, seven days a week

Provides assessment, treatment, and rehabilitation for adults with schizophrenia and other psychosis disorders. In partnership with clients, families and other service providers, assists clients with living, learning, work and social needs.

Services include:
  • Assessment - psychiatric and psychosocial
  • Community consultation, development and education
  • Clinical case management (residents of Peterborough County only)
  • Family education and support
  • Medication management
  • Psychiatric treatment and follow-up
  • Psychosocial treatment
  • Psychoeducation
  • Psychosocial programs (e.g. peer support group, youth group etc.)
  • Rehabilitation (e.g. vocational, employment assistance)
  • Wellness management

Hours Vary by each Psychosocial Program
Core service: Mon-Fri 9 am-5 pm
Flex time based on need
Areas Served Haliburton County ; Kawartha Lakes ; Northumberland County ; Peterborough (County of)
Eligibility Individuals with a diagnosis of schizophrenia or related disorder 
Clinical case management: Residents of Peterborough City and County only 
18 year(s) and up;  
Patients under the age of 18 will be referred to the Lynx, early psychosis intervention program.
Application Referral from the person needing assistance, community agencies, physicians or family members
Languages English
Fees None - No fees for people with a current OHIP card
Bring OHIP card to every appointment
