Description
| Description (Service) | Assists developmentally disabled adults living substantially on their own in the community to live as independently and as safely as possible. Individuals living with family or in residential programs may also be eligible to receive supports for periods of up to one year if they either require some assistance with advocacy or wish to plan to live more independently and require case management supports to achieve their goals. Services include:
