| Eligibility | Must be a resident of Ontario; have an annual net income of $22,200 or less for a single person, or a combined annual net income of $37,100 or less for a couple; have no access to any other form of dental benefits (e.g., through private insurance or other government programs) Ages: 65 year(s) and up; |
| Application | Self referral. Call 705-743-1000, ext 265 for an intake appointment or for information. |
| Languages | English; French - Publications; Interpretive Services - Interpretation; Other - Interpreter services can be provided for other languages. |
| Fees | Fees dependent on service. Co-payment for dentures - subject to change. Call the clinic for details. |
