Description

Description (Service) Urgent/emergent clinic provides comprehensive assessments, recommendations, treatment plans, education, support and care.
  • For older adults with complex heath concerns about memory loss or confused thinking, walking and mobility, falls or risk of falls, bladder and bowel control, multiple medications, difficulty coping, safety issues, frequent hospital visits
  • Interprofessional team includes nurse practitioners, physiotherapists, occupational therapists, social workers, pharmacists, Home and Community Care case managers and specialized physicians when required.
  • To come prepared to their appointment seniors should bring: walking aids, eye glasses, hearing aids and all medications including over-the-counter products. Appointment may take two to three hours.
  • Also offers appointments at home for clients who have difficulty attending the clinic.

Eligibility High-risk seniors generally 65 years and older
Does not treat seniors who live in long-term care facilities
Application Self referrals welcome and a referral signed by a physician or nurse practitioner. Referral form available on hospital website
Languages Interpretation services when required
