Placement services determines eligibility for admission and prioritizes and manages the admission process to long term care facilities.

|Eligibility||Must be a resident of Ontario who is 18 years of age or older. Must have a valid Ontario Health Insurance Plan (OHIP) Card. Further eligibility criteria will be assessed upon initial intake.|
|Application||Assessment required Valid Ontario Health Insurance Plan (OHIP) number is required.
Waiting Period: No waiting period to obtain placement information but waiting periods vary for admission to each long term care facility.

|Fees||No fee for assessment/application but fees associated with admission to Long Term Care Facilities as set-out by Ministry of Health and Long Term Care.
Reductions in fees for basic accommodation according to criteria set out by the Ministry of Health and Long Term Care
