100 bed Long-Term Care facility for people who can no longer be cared for at home or in an assisted living facility due to illness, injury, fragility, or other limitations. Suitable for patients who may have suffered from a prolonged acute or chronic illness or may have recently been injured or undergone surgery with stable conditions but requiring ongoing assessment and treatment.

• Provides a range of rehabilitative services including assisting patients with physical, cognitive, psychosocial and/or communication impairments.
• Rehabilitation services include respiratory, physical, occupational, and speech therapies.
• Alzheimer's and related dementia program enhances quality of life through supportive care and success-oriented activities that help maintain self-esteem and physical, cognitive, and psychosocial abilities.
• Structured group activities provide routine while small groups and one-to-one sessions help meet more individual needs
