Description (Service) | The Geriatric Emergency Management (GEM) Advanced Practice Nurse provides targeted/comprehensive assessments for at risk older adults presenting to the Emergency Department (ED).

Eligibility | Clients (usually age 75 or older) generally have recently experienced a functional or cognitive decline, have trouble managing medications, have experienced falls or are at risk for falling, or are living alone. |
Application | Referral Required. Seniors may be referred from their primary care provider office, ED staff or physicians, or may be sent in for assessment by community partners such as the GAIN team, PASE, Retirement homes, Long-Term Care homes or other hospitals. |
