| Description (Service) | Family-centred care program designed to assist those diagnosed with a terminal illness or a life limiting illness (chronic disease) * Support is provided to individuals and families in home, hospital or nursing home * Volunteers provide companionship, assistance with light household duties, and simple comfort measures Bereavement support and grief programs include one-on-one support, monthly caregiver-support groups, and ongoing social support groups. Any individual who has suffered a significant loss is welcome to bereavement support Respite service provided for family members who are caring for a love one * advanced care planning workshops and presentations available to groups A visiting hospice - no beds provided |
