Description (Service) A client-centred, recovery-oriented mental health service.
  • Facilitates community living, psychosocial rehabilitation and recovery of persons who experience serious mental duress and functional impairments, and have not benefited from traditional out-patient programs due to their complex needs. 
  • Provides: Treatment rehabilitation and supportive services; Medication management and education; Intensive case management and individual supportive therapy; Small client to clinician ratio; Ability to intensify services directed by individual client needs; Rehabilitation and recovery focus; Vocational support; Addictions counselling; Occupational Therapy; Therapeutic relationship building and creative outreach; Advocacy and partnership.

  • Clients with severe mental illness that seriously impairs their functioning in community living.  
  • Eligibility criteria identified through the Ontario ACT Association
