Description (Service)

A client-centred, recovery-oriented mental health service.

  • Facilitates community living, psychosocial rehabilitation and recovery of persons who experience serious mental duress and functional impairments, and have not benefited from traditional out-patient programs due to their complex needs.

Provides:

  • Treatment rehabilitation and supportive services
  • Medication management and education
  • Intensive case management and individual supportive therapy
  • Small client to clinician ratio
  • Ability to intensify services directed by individual client needs
  • Rehabilitation and recovery focus
  • Vocational support
  • Addictions counselling
  • Occupational Therapy
  • Therapeutic relationship building and creative outreach
  • Advocacy and partnership

Hours Mon-Sun 8 am-4 pm
Areas Served Northumberland County
Eligibility
  • Clients with severe mental illness that seriously impairs their functioning in community living.
  • Eligibility criteria identified through the Ontario ACT Association.
