  • hospitals
  • long-term care homes
  • Local Health Integration Network centres
  • community support services
  • community mental health and addictions services
  • community health centres.
  • Work with people to identify goals for care
  • Help people transition from hospital to home
  • Support people to remain in their homes and communities
  • Provide people with access to supportive care settings
