Public BulletinJanuary 26, 2012 - Service Ontario offers Seniors over the age of 80 a mail-in renewal service for their health cards where they can apply for their new health card by completing and signing the back of their renewal notices and returning them in a pre-addresses envelope * seniors who prefer to renew their health cards in person may still do so at 34 Simcoe St, 320 Bayfield St and 274 Burton Ave
