Description (Service)

Goals of program are to increase patients' sense of control over illness and prevent hospitalization

  • provides medication, symptom monitoring, wellness promotion, community service awareness, education and psychosocial support for patients who have a diagnosis of schizophrenia or schizoaffective disorder who have demonstrated inadequate response treatment and/or have intolerable side effects, and need to be stabilized on new or current medications
  • patients will be transitioned back to the care of family physician (GP) or former psychiatrist after approximately 6 months of education and monitoring

  • formal diagnosis of schizophrenia or schizoaffective illness
